Colchester Medical Practice Primary Care Network (CMP PCN)

A Primary Care Network (PCN) is a group of local healthcare providers, such as general practitioners (GPs), nurses, pharmacists, and other specialists, working together to improve patient care in a specific community. By collaborating, PCNs enhance access to services, provide more personalized care, and support better management of long-term conditions.

Colchester Medical Practice PCN employs additional staff across all of our sites. These are comprised of-a GP, Emergency Care Practitioners (Paramedics), Pharmacists, Pharmacy Technicians, Physiotherapists, Occupational Therapist, Health and Wellbeing Coaches, Care Coordinators, GP Assistants as well as a Digital and Lead to assist with the delivery of services.
